A blank LSTA Special Project Report form can be found on the MBLC Web site at mblc.state.ma.us/grants/lsta/manage/. Copies are available in Microsoft Word format. If you wish to fill out the form in word processing, download the Microsoft Word version of the form. Instructions are provided on the site. You will need to fill in you library name, project name and project number. Please don't forget to send us the narrative section of your report electronically, as mentioned above.
